Our new patient forms are available on our new patient portal. In order to get access to this portal, please text our office at 980-939-3609 to inquire about setting up a new appointment with us! Once you are in the portal, please click on Forms and Questionnaires and fill out the following under “New Patient Forms”

Also please update all your demographic information, mainly address, phone number, picture, and all other information. Insurance does not cover our services, so you do not need to put in any insurance information.

 

Please also upload any recent labs you have had done within the last year. We will review your labs and determine which additional labs you need before your appointment is scheduled. Labs are covered through insurance if your plan covers labs.
